Case Taking Proforma has been diligently designed by the practicing Dr. Salman Saeed, to serve as a reference book, and as a guide to the process of history taking and physical examination. Divided into two parts, the first part deals with history taking followed by physical examination in the second part of the book. It attempts to prime up your clinical skills and documentation. It will also come in handy for quick reference while gathering information. Though, it doesn’t aim to replace any standard clinical methodology textbook, nevertheless it can be categorized as a workbook to establish and streamline your comprehensive case taking skills. Towards the end, there’s a brief topic on Cardio-Pulmonary Resuscitation (CPR), that covers the basic parts of CPR. This section has been adapted from the American Heart Association.
